1. Initial Contact
We welcome referrals from health professionals, including nurses, allied health practitioners, and general practitioners. Please provide a brief overview of the client's needs and any specific care requirements.
2. Assessment & Matching
Our care managers will conduct a thorough assessment to understand the client's lifestyle and health status. We will then match them with a professional caregiver who shares their values and is the best fit for the home environment.
3. Onboarding & Support
Once a caregiver is assigned, we provide comprehensive onboarding and training. We also offer ongoing support for both the caregiver and the client to ensure a smooth transition and long-term success.
